Jacob Faltin-Medina, a cherished member of the Ottawa community and a dedicated school psychologist, bid farewell to the world on Sunday, January 14, 2024, at the age of 29. His untimely departure has left a profound void in the hearts of those who knew him.
A Celebration of Life is planned on Saturday, January 20, 2024, from 12 to 3 p.m. at the Mueller Funeral Home in Ottawa. Family and friends will gather to honor and reminisce about Jacob’s beautiful journey through life.
Born on May 10, 1994, in Oak Lawn, IL, to Ronald and Sherri Faltin, Jacob spent his formative years in Plainfield. After earning a Bachelor’s Degree from the Chicago School of Professional Psychology, he devoted his professional life to making a positive impact as a school psychologist for L.E.A.S.E. His students affectionately knew him as Mr. Jacob, a beloved figure who left an indelible mark on their lives.
On August 19, 2021, Jacob exchanged vows with his life partner, Mario Saldana, in a ceremony held in Ottawa. Their union stood as a testament to love, commitment, and shared dreams. Together, they found joy in life’s simple pleasures—spending time with friends, attending festivals and concerts, expressing joy through song and dance, and exploring the wonders of the world through travel.
Jacob is survived by his devoted husband, Mario; parents, Ronald and Sherri; and a sister, Mary “Katie” Lesko. He was preceded in death by his maternal and paternal grandparents.

The LaSalle County Coroner’s Office confirmed that the person who tragically lost their life in a house fire on Sunday evening in Ottawa was 29-year-old Jacob R. Faltin-Medina. The initial autopsy results revealed that Jacob succumbed to smoke inhalation and carbon monoxide poisoning resulting from the blaze.
The fire, which occurred in the 400 block of 2nd Avenue, remains under investigation. The Ottawa Fire Department indicated that it appears to be accidental.
Firefighters responded to the call around 5:20 p.m. and located Jacob on the second floor amidst heavy heat and smoke conditions. Despite their efforts, he was unresponsive. Jacob was then taken to an area hospital, where he succumbed to his injuries. Firefighters spent about half an hour bringing the blaze under control.
Jacob R. Faltin-Medina’s passing has cast a shadow over the Ottawa community. A Celebration of Life has been planned to allow friends and family to come together and honor the memory of this beloved psychologist who touched the lives of many.
